Immigration Physical
What is an immigration physical exam?
To apply for citizenship in the United States or adjustment in status, you will need to complete a medical exam. This is a routine part of the immigration process. The exam and subsequent completed report (Form I-693) are overseen by physicians designated by the United States Citizenship and Immigration Services (USCIS). These physicians are known as Civil Surgeons.
Capitol Travel Medicine provides Immigration Physical Exams for both adults and children.
What happens during your exam?
The process is completed in two visits, 1- 2 weeks apart. During the first visit, you will be asked questions about your medical history and a physical exam will be performed. We will give you a lab order to get tested for:
- Tuberculosis
- Syphilis (ages 18-44)
- Gonorrhea (ages 15-24)
Our office will advise you if vaccines are needed. Vaccine prices at Capitol Travel Medicine are comparable to pharmacy rates.
During the second visit, you will receive the Form I-693 signed by the Civil Surgeon. Your original Form I-693 will be provided to you in a sealed envelope. You will also be given a copy along with your lab results. Applicants are responsible for submitting the original Form I-693 to USCIS.
What is the cost?
Please be aware that these visits are not covered by insurance.
Base cost: $495.00
- The base cost INCLUDES physical exam and basic labs (TB-IGRA, RPR and GC).
- Additional charges apply if vaccines or follow-up lab/X-rays are needed.
- We do not accept insurance for INS physicals. You will receive an itemized receipt at check-out.
What should you bring to your immigration physical?
These are the documents you MUST bring to your first visit:
- Valid Passport or government issued photo ID
- Immunization/Vaccination Records
- Lab results (if any) regarding immunity to certain diseases (measles, mumps, rubella).
